Abstract

Routine general practice (GP) care is rarely comprehensively described in clinical trials. This paper examines routine GP care within the lifestyle approach to managing panic (LAMP) study. The aim of this paper is to describe/discuss routine GP care for panic disorder (PD) patients within both study arms in the LAMP study. An unblinded pragmatic randomised controlled trial in 15 East of England GP practices (2 primary care trusts). Participants met Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition criteria for PD with/without agoraphobia. Follow-up measures recorded at 20 weeks/10 months following randomisation. Control arm, unrestricted routine GP care (practice appointments, referrals and prescriptions). Trial arm, occupational therapy-led lifestyle treatment comprising lifestyle review of fluid intake, diet pattern, exercise, caffeine, alcohol and nicotine. Primary outcome measure: beck anxiety inventory. At baseline, participants attended 2-3 times more GP appointments than population average, reducing at 10 months to 1.6 times population average for routine GP care and 0.97 population average for lifestyle arm. At 10 months, 33% fewer referrals (6 referrals; 0 mental health) than at baseline (9 referrals; 2 mental health) were made for lifestyle arm patients compared with 42% increase (from 12 referrals; 8 mental health at baseline to 17 referrals; 7 mental health) in GP care arm. Selective serotonin reuptake inhibitors were prescribed most often. Benzodiazepines and beta-blockers were prescribed more often than tricyclic against current clinical guidelines. In conclusion, we found that PD patients at baseline were high healthcare resource users. Treatment in both study arms reduced resource use. Routine GP care requires further review for this patient group.

Abstract

Current treatment guidelines for anxiety disorders, including panic disorder (PD), recommend either medication or cognitive behavioural therapy (CBT). There is currently a call through the Layard Report for significant investment to increase the availability of CBT resources. However, there are reported limitations to both medication and CBT in the treatment of anxiety, and it appears prudent to consider additional methods of treatment that may offer effective interventions. One such intervention is based around the evidence of altered sensitivity within a number of physiological body systems in anxiety patients (particularly those with PD), all of which are influenced in their function by habitual lifestyle behaviours. A randomised controlled trial compared a 16‐week occupational therapy‐led lifestyle intervention and routine general practice (GP) care for PD. At 20 weeks, 14 symptoms with ‘moderate’ to ‘very severe’ ratings were assessed in 36 GP and 31 lifestyle‐intervention patients. Composite symptom profiles, similar at baseline, were produced. The GP intervention produced modest improvements in most symptoms. The lifestyle intervention overall produced greater symptomatic relief (Wilcoxon signed ranks test, P= 0.008). The physiological and cognitive symptom profile also changed more with lifestyle intervention. Occupational therapists have developed their interventions based on their understanding of everyday occupation. Habitual lifestyle behaviours are characterised as being recurrent elements of everyday occupation and are, therefore, legitimate targets for occupational therapy interventions. They provide a vehicle through which to encourage patients to regain understanding and control of their own anxiety symptoms.

Abstract

School library media specialists (SLMSs) often struggle with assuming leadership roles. Discrepancies existed in perceptions of SLMSs of their leadership preparedness, their opportunities to exert leadership, and their assumption of leadership roles. The purpose of this quantitative study was to explore the perceptions of SLMSs regarding their instructional leadership and to examine the extent to which they practiced instructional leadership. The study was designed to determine whether there were differences between SLMSs perceptions of the importance of their leadership roles and their opportunities to practice those roles. The results of the study indicated that SLMSs perceived all of the leadership roles to be more important than they were able to carry out in practice and that supportive administrators were the most essential factor in providing SLMSs the opportunity to practice and expand their roles as instructional leaders.

Abstract

Purpose

The purpose of this paper is to underpin a scoping study commissioned by community leaders to assess the potential for creating a “health café” in the centre of Boston, in eastern England, UK, to facilitate healthier lifestyles.

Design/methodology/approach

A mixed methods and framework analytic approach was adopted, using documentary, focus group, interview and survey data. The paper drew on social marketing principles to enhance the community relevance of findings.

Findings

Community stakeholders and public were generally supportive of a “health café” facility in the town centre. Accessibility and a welcoming environment were seen as key factors. A wide range of health-related services in addition to providing healthy foods were proposed. Key issues identified were: a wider role of the facility as a community “health hub”; appropriate marketing approaches; food provision issues and sustainability. All groups contacted saw the word “health” as off-putting.

Research limitations/implications

As with many commissioned scoping studies, the timetable for delivery was very short, just three months, significantly influencing the choices of methodological approaches taken up. This made it important to provide a multi-disciplinary multi-methods design to enhance triangulation and a research team with extensive community research experience including previous research in this region. It was also important to specify and locate any knowledge claims from the findings.

Practical implications

The research helped engage community stakeholders to tap a diversity of views which could be adopted by community leaders into their ongoing health strategies and development plans for a “health hub” for Boston.

Originality/value

The paper provides important information for those embarking on community health education projects and particularly in how to tailor health research methods to real-world timescales and stakeholder perspectives. Insights are also provided into community attitudes, understandings and behaviours towards healthy living in a part of the UK with a well-documented history of poor health.

Abstract

Purpose

This study aims to examine empirically the existence of four classical and four emerging buyer–supplier relationship (BSR) types and how they differ in terms of behavioral dynamics and performance measures.

Design/methodology/approach

This study uses an online survey to collect data from 371 purchasing managers in the USA.

Findings

A cluster analysis statistically supports the existence of five of these eight BSR types, including strategic/bilateral partnership, market/discrete, supplier-led collaboration, captive supplier/buyer dominant and captive buyer/supplier dominant BSRs. Further, ANOVA tests show that these five BSRs differ in terms of behavioral outcomes and performance measures.

Research limitations/implications

This study is based on a cross-sectional survey so it cannot examine how these BSR types may evolve over time, and it is not suitable to examine some rare types of BSRs. In addition, this study does not consider contextual factors that may moderate the influence of BSR types on the behavioral dynamics and performance measures.

Practical implications

Managers should consider the potential to be able to develop and enhance a strategic/bilateral relationship with their supply chain partners, which in at least some circumstances can lead to superior performance results. Similar observations can be made with respect to supplier-led and, to a lesser degree, buyer-led collaboration.

Originality/value

Most existing research of the BSR types is largely a product of theoretical classifications, and there is also a lack of research of their performance implications. This study fills these gaps in the literature.

Abstract

Purpose

Integrated supplier management (ISM), new product development (NPD) and knowledge sharing (KS) practices are three primary business activities utilised to enhance manufacturers' business performance (BP). The purpose of this paper is to empirically investigate the relationships between these three business activities (i.e. ISM, NPD, KS) and BP in a Taiwanese electronics manufacturing context.

Design/methodology/approach

A questionnaire survey is first administered to a sample of electronic manufacturing companies operating in Taiwan to elicit the opinions of technical and managerial professionals regarding business activities and BP within their companies. A total of 170 respondents from 83 companies respond to the survey. Factor, correlation and path analysis are undertaken on this quantitative data set to derive the key factors which leverage business outcomes in these companies. Following empirical analysis, six semi‐structured interviews are undertaken with manufacturing executives to provide qualitative insights into the underlying reasons why certain business activity factors are the strongest predictors of BP.

Findings

The investigation shows that the ISM, NPD and KS constructs all play an important role in the success of company operations and creating business outcomes. Specifically, the key factors within these constructs which influenced BP are: supplier evaluation and selection; design simplification and modular design; information technology infrastructure and systems and open communication. Accordingly, sufficient financial and human resources should be allocated to these important activities to derive accelerated rates of improved BP. These findings are supported by the qualitative interviews with manufacturing executives.

Originality/value

The paper depicts the pathways to improved manufacturing BP, through targeting efforts into the above‐mentioned factors within the ISM, NPD and KS constructs. Based on the empirical path model, and the specific insights derived from the explanatory interviews with manufacturing executives, the paper also provides a number of practical implications for manufacturing companies seeking to enhance their BP through improved operational activities.

Abstract

Purpose

The purpose of this paper is to explore what an idiosyncratic and dynamic sense of belonging entails for consumption in a lifestyle sport, an ever shifting and progressing world in which individuals engage in community while also seeking to individuate their own sense of belonging.

Design/methodology/approach

The authors adopt an ethnographic approach in the context of a regional skateboarding community. Over a year at local skateparks, we interviewed 15 well-established, committed members of the community identified by others (through snowball techniques) to allow us to delve into the phenomenon. These interviews were conducted as part of the primary author's doctoral thesis (See Harris, 2011).

Findings

An idiosyncratic and dynamic sense of belonging is prevalent in the lifestyle sport community, even among well-established members. This is reflected in and motivated by a variety of consumption, as well as overconsumption practices.

Practical implications

Understanding the idiosyncratic and dynamic nature of a sense of belonging allows marketers to design offerings to effectively deal with the ambiguities of belonging but also raises the potential for the destructive use of marketing.

Originality/value

The authors demonstrate how approaching belonging through a dynamic and idiosyncratic sense of belonging provides a deeper understanding of belonging and related consumption activities in a lifestyle sport.

Abstract

Flexible scheduling in school libraries is supported by the American Association for School Libraries (AASL) and the Association for Educational Communications and Technology (AECT). Support is based on the constructivist theory of learning and posits increased learning, collaboration, and visitations by classes, small groups, and individuals to the availability of resources during the time of need, yet there is no direct evidence to support flexible scheduling. The quantitative study sought to examine the relationship between media center scheduling on students’ academic achievement, teacher and media specialist collaboration, and class visitation in an elementary school. The researcher utilized an experimental posttest-only control group design. The point-biserial correlation was utilized to identify any relationship between groups who utilized the media center on a fixed versus a flexible schedule and criterion-referenced test scores. No significant relationship was found between scheduling patterns, student achievement, and collaboration. However, the research supported increased number of visitations by classes on a fixed schedule.

Abstract

Purpose

The purpose of this paper is to evaluate the use, attitudes and perceptions of both teachers and students towards social media platforms (Facebook and Twitter) in healthcare higher education practice.

Design/methodology/approach

Systematic literature review. A search of published and unpublished databases was conducted from January 2000 to January 2013. Data were synthesised through a narrative approach. All included studies were critically appraised using Critical Appraisal Skills Programme appraisal tools.

Findings

From a total of 876 papers, 16 publications met the eligibility criteria. The review papers were largely of moderate methodological quality. The results indicated that Facebook and Twitter are perceived as potentially useful adjuncts in healthcare higher education practice. Their use as part of a “blended” approach to classroom teaching has been employed to enhance communication and increase accessibility of students to real-world practices and expertise. Whilst students perceive this to be of value, some have reservations regarding the possibility of faculty members spying or infringing on their “virtual” social community. Faculty members expressed some reservations regarding the perceived boundary between student-staff relationships.

Originality/value

This is the first paper to systematically review the literature on the use of social media networking sites in health education. This identified both the potential strengths and weaknesses of using this new technology, and highlights areas to expand knowledge with further research and practice.

Abstract

Purpose

The purpose of this paper is to study the influence of two climates for innovation constructs, namely, leadership and organisational culture, on workplace innovation and career satisfaction.

Design/methodology/approach

This study used structural equation modelling to test the data from 3,125 engineering professionals in the Australian Public Service (APS).

Findings

The structural model indicated that leadership for innovation and ambidextrous culture for innovation influenced workplace innovation which, in turn, improved career satisfaction. Moreover, modelling revealed a significant relationship between ambidextrous culture for innovation and career satisfaction. This study also investigated mediation effects and revealed both simple and sequential mediation paths in the model. It was found that improving workplace innovation and career satisfaction through recognition of an engineer’s contribution to their agency would assist in retaining and advancing in-house engineering expertise.

Practical implications

The structural model could be used to address current shortages of engineering professionals in the Commonwealth of Australia departments. The findings emphasise the importance of Commonwealth departments providing opportunities for their engineers to engage in creative and innovative projects which enhance their professional career.

Originality/value

This study fills the gap in the innovation literature by exploring the relationships through which socio-psychological factors affect workplace innovation and career satisfaction on the innovation process for engineering professionals in the APS.
